The decision to move from ones home to a new environment is neither easy or simplistic. Whether the decision is taken at a point when clients can be actively involved or when health and well – being are impaired, moving home can entail a challenging and stressful process. Even where there is a robust family support network, the magnitude of such a task can be overwhelming and time consuming. Epiphany can take on the associated challenges and alleviate the stress for all involved by undertaking the necessary tasks to facilitate a smooth transition. Whether clients are downsizing, moving in with family or moving into a care home or supported accomodation, we can provide effective solutions to assist clients and their support networks. Epiphany can provide as much or as little help as required to ensure the process is undertaken with minimum disruption to all. We can facilitate the packing, removal and transportation of all furniture, to the new home location as well  engage the intervention of an occupational therapist should complex aids and or adaptations be required. We can assist with GP registration and set up repeat prescription deliveries from local pharmacy. Our ultimate objective is to facilitate a move in line with the expressed wishes and preferences of our clients and  ensure welfare and well-being.

The decision to move into a care home be it a residential or nursing home, or supported living scheme can be a daunting prospect for our clients as well as their support networks. Once the decision has been made that a care home is the very best and most appropriate option, it is important to facilitate a move that is specifically catered  for the needs of the individual involved. At Epiphany we recognise that facilitating a smooth transition into a care home is critical, to allaying the fears and anxieties often associated with such a move, not just for our clients but for their support networks also. At Epiphany we  specifically cater to the specific needs of each client and this particularly important for clients diagnosed with dementia so as to alleviate exacerbation of confusion. Ensuring the chosen care home has a support plan clearly outlining how care is to be delivered to the vulnerable adult is important to ensure care is delivered in a personalised manner that is in line with their personal preferences and wishes. From the practicalities of ensuring and or facilitating the labelling of clothing and belongings to negotiating costs in line with client needs, to undertaking reviews of care provision as and when required, we can provide all the necessary advice and information to identify the best care home for each resident and continue to provide support and advise as required throughout the care journey.
